The following three surveys provide examples of the range of capabilities of our survey products.

1.	Simple six-question survey	This survey provides a simple example of survey capabilities - validation of data entry (email address and numeric values); enabling and and disabling questions based on responses to a question (Question 2); alert messages and auto-skipping to another question (Question2 - "No" skips to Question 6); and on-form calculations (Question 5). 2.	Sample Survey to Demonstrate Survey Capabilities	The "Sample" survey demonstrates many of the features and functions of surveys created by our survey program generator. These include the ability to specify HTML code in the survey specifications, required fields, enabling and disabling of fields based on survey responses, validation of numeric data entry, calculations, and displaying messages when radio buttons are selected. 3.	Community College Housing Survey	This survey is based on a survey generated for a client who surveys students housing needs. It includes all of the features and functions listed above, plus images of various apartment designs. (Note that the survey may take a while to load, as there are 26 questions and four images.) In addition to the survey itself, many other forms may be needed both for the survey (login, initial form, response completed form, etc.) and for survey administration (sending out emails, reviewing the results, etc.). Chesapeake Data Systems provides a standard set of such forms, which can be easily modified (if needed) to meet your specific requirements.
